Karne Hesketh’s injury time try ended a winless run for the Japanese at the World Cup dating back to 1991.
South Africa had led 12-10 at half-time through tries from Francois Louw and Bismark du Plessis.
Lood de Jager and Adriaan Strauss then scored to keep the Springboks ahead, but Ayumu Goromaru contributed 24 points before Hesketh crossed over deep in stoppage time.
